HammondCare in Australia is seeking a Dementia Advisor/Consultant to provide expert guidance and emotional support to people living with dementia and their families. This full-time role at our Scone Site focuses on applying dementia-specific clinical knowledge in an aged care setting and mentoring care staff to lift care quality.

You will collaborate with teams across the organisation, lead practice development, and participate in resident care planning, supervision, and quality improvement

Qualifications

• 3 years of proven dementia experience.
• Registered Nurse qualification is essential.
• Desirable: Training and Assessment qualification.
• Strong teaching, mentoring and coaching skills.
• Able to perform personal care and resident support.
• Strong communication and interpersonal skills.
• Strong administrative and computer skills.

Responsabilités

• On the ground support providing dementia-specific expert advisory services within HammondCare services, through participation in regular clinical review meetings; coaching staff and building their capacity to handle dementia-related situations.
• Build staff capacity through education, mentoring and coaching; spend time with the team on the ground and lead by example in care support, plus case study discussions and review of documentation.
• Plan engagement with individuals and teams to achieve agreed practice development objectives and quality improvements.
• Participate, contribute and learn as part of the Residential team in team meetings, professional supervision and team activities.
• Evaluation and reporting by using HammondCare processes to communicate, monitor and manage work delegations and projects including evaluation and reporting.
• Participating in projects and behaviour management planning.
