Job summary
Nova Scotia's Department of Opportunities and Social Development seeks an Intensive Planning and Support Coordinator (IPSC) in Sydney, NS. This term position focuses on person-directed planning and safeguarding the rights of people with disabilities, working with multidisciplinary teams to build individualized support plans.
You'll support transitions from institutions to community life, coordinate services, and advocate for dignity and choice, guided by trauma-informed, culturally competent
Qualifications
- Licensed professional (RN/LPN, Social Work, Occupational Therapy, Physiotherapy or Nutritional Studies) with active registration.
- Valid driver’s license and reliable transportation; accommodations available.
- 5 years of related experience.
- Ability to develop, implement, evaluate comprehensive support plans.
- Strong communication and collaboration across teams.
Responsibilities
- Provide intensive, personalized support and planning for individuals with disabilities.
- Build trusting relationships and connect people to services and professionals.
- Support moves from institutional settings to community life.
- Balance priorities while coordinating with providers and agencies.
- Develop inclusive, person-centred plans with natural supports.
- Advocate for rights and person-directed planning.
