Job summary

commUnity+ is a community led organisation delivering integrated services across Melbourne’s western suburbs. The Social Worker will support the Any Front Door initiative, providing targeted, short term assistance and ongoing case management to vulnerable clients.

The role operates across several sites with outreach, requires a tertiary Social Work qualification, AASW membership, strong cross-cultural communication, and a valid driver’s license.

Qualifications

• Tertiary qualification in Social Work or equivalent.
• Membership of, or eligible for membership of, the Australian Association of Social Workers (AASW).
• Experience in case management and social work with people experiencing disadvantage.
• Experience or knowledge developing internal referral pathways and systems.
• Highly developed interpersonal and cross-cultural communication skills.
• Ability to manage a large and diverse caseload and work under pressure.
• Ability to work independently at offsite outreach locations and manage mobile service delivery.
• Proficiency with Microsoft Office, Zoom, and Microsoft Teams.
• Driver’s licence and ability to travel between offices and outreach locations; Working with Children Check and National Police Check.

Responsibilities

• Provide targeted, short-term assistance and ongoing case management.
• Conduct assessment, service engagement, referrals, advocacy and coordination of supports.
• Coordinate holistic services addressing legal and non-legal needs.
• Work across multiple sites and outreach locations to support service delivery.
• Contribute to service design, collaboration and continuous improvement.
