Job summary

Wellways Connected Care in Northwest Tasmania seeks a welcoming Intake Worker to be the first contact for people accessing our services. You will support intake, screening and assessment to understand needs and enable timely access to care, working with a multidisciplinary team.

You will apply trauma-informed, recovery-oriented approaches, promote informed choice and cultural safety, and coordinate with partner organisations to ensure appropriate referrals and safe escalation when needed.

Qualifications

We recognise that diverse experiences and perspectives strengthen our workforce and the communities we support.** **Join us to support a multidisciplinary clinical team across Northwest Tasmania*** **Shape high-quality, person-centred mental health care through Connected Care*** **SCHADS 3B ($40.49- $43.42p/h) + Super + Salary packaging options****About the role**Connected Care is seeking a welcoming and passionate **Intake Worker** to be first point of contact for people accessing our Connected Care centre across Northwest Tasmania.You will support intake, screening and assessment, including use of the Initial Assessment and Referral Decision Support Tool (IAR-DST), to understand participant needs, identify appropriate levels of care and support timely access to services.**About Connected Care**Connected Care is a statewide, integrated mental health service delivered by Wellways in partnership with Stride Mental Health and Clarity Health Care and commissioned by Primary Health Tasmania (PHT).Supporting adults aged 18 years and over, Connected Care provides free mental health care and support through a 'no wrong door' approach and stepped-care model.
