Job summary

Wellways is launching Connected Care across Tasmania and seeks a Service Integration Coordinator to build a statewide network. You will map health and community services, establish referral pathways, and foster strong collaborations with local organisations to support adults needing coordinated mental health care.

You will work with diverse communities, attend interagency networks, and contribute to resources and reporting.

You'll map health and community services, connect with local organisations, support community engagement and maintain the Connected Care Support Catalogue.This is a practical, relationship-focused role with variety and the opportunity to help shape how a new 'no wrong door' service works across Tasmania.About Connected CareConnected Care is a statewide mental health service delivered by Wellways in partnership with Stride Mental Health and Clarity Health Care and commissioned by Primary Health Tasmania.The service supports adults aged 18 years and over to access free, coordinated mental health and wellbeing support, using a stepped-care approach that responds to their individual needs and goals. A multidisciplinary team brings together clinical, psychosocial and peer expertise to support people to access the right care and move between levels of support as their needs change.About the OpportunityThis role will help build the foundations for Connected Care across Tasmania. Working with the Service Lead and Connected Care team, you'll:Map and maintain information about health, mental health and community servicesDevelop and maintain referral pathways and relationships with key organisationsAttend community and interagency networks and deliver information sessionsSupport engagement with diverse and underserved communitiesContribute to resources, reporting and the evaluation of referral pathwaysYou'll work across a broad network including mental health services, general practice, primary and allied health, community organisations, housing and homelessness services, family and carer supports, and services supporting diverse communities.The role can be based in Tasmania's North or North-West, with regular travel across the region and some travel elsewhere in Tasmania.What you bring:You'll bring experience building strong working relationships across health, mental health or community services, with the ability to connect people and organisations and support the development of practical referral pathways.You'll have an understanding of community development and recovery-oriented, trauma-informed and strengths-based approaches, along with knowledge of the broader services that support people's mental health and wellbeing.You'll also bring:A relevant tertiary qualification, or a Certificate IV/Diploma with relevant experienceConfidence engaging with a wide range of stakeholders, including presenting information to groupsAn understanding of culturally safe and inclusive practice and the diverse communities accessing health and community servicesAn understanding of family and caring roles and their impact on carers' wellbeingStrong organisation skills and the ability to manage your own work and deliver against an agreed planThe ability to gather, interpret and use information and feedback to identify issues and opportunitiesConfidence preparing clear written information, resources and guidesGood Microsoft Office skills and the ability to learn new systemsA willingness and ability to travel throughout TasmaniaExperience working within the Tasmanian health and community service system, or developing service directories, resource guides or referral pathways, will be highly regarded.Personal lived experience of mental health challenges, or caring for someone who has, is also welcomed.Compliance & ScreeningYou will need to meet Wellways' required compliance and screening checks including:Right to work in Australia100 points of IDValid Driver Licence and willingness to travelSatisfactory National Police Record CheckInternational Police Check (if appliable)NDIS Worker Screening CheckNDIS Worker Orientation Module Certificate - Quality Safety and YouCurrent Working with Vulnerable People CheckWhy Join Wellways?At Wellways, we believe people are the experts in their own lives.
