Job summary

TLC First Support Services is seeking an experienced Intake Service Manager to lead end-to-end intake, referrals and onboarding for participants under the NDIS framework. You will act as a primary contact for participants, families and partners, ensuring prompt, compliant and compassionate service delivery.

The role focuses on coordinating onboarding, consent, handovers to delivery teams and maintaining accurate records while improving intake processes and referral pathways to maximise

Qualifications

• Experience in NDIS, disability, aged care or similar intake/service coordination roles.
• Confidentiality and participant-centred practice understood and applied.
• Experience using CRM, rostering, case management or client record systems desirable.

Responsibilities

• Manage participant enquiries, referrals and intake documentation.
• Complete initial screening and review NDIS plans, funding and service suitability.
• Coordinate onboarding, service agreements and consent forms.
• Maintain accurate intake records, CRM notes and referral tracking.
• Liaise with participants, families, Support Coordinators and Plan Managers.
• Monitor intake timeframes and resolve barriers.
• Contribute to community engagement and referral pathway development.
