UnitingCare Community on the Gold Coast is seeking an Intake Practitioner to be the first contact for callers and walk-ins seeking housing support. You will screen, triage and assess risk, then refer to case management or accommodation services to help clients secure housing and services.

The role involves daily setup of the Gold Coast Homeless Hub, managing inquiries across channels, and maintaining client data in SHIP and QHIP. A rewarding, frontline position in a not-for-profit setting.

Qualifications

• Minimum Cert. IV in human services or related field (Tertiary qualification in human services or related field highly desirable)
• Current Drivers Licence, Positive Notice Blue Card
• Proficiency in Information Technology systems (MS Word, Excel, Outlook)
• Demonstrated experience in the community services sector
• Ability to engage with people in crisis with complex needs
• Ability to engage with a diverse range of people and display culturally competent practice
• Ability to work collaboratively as part of a team as well as autonomously
• Commitment to learning and professional development including supervision practices

Responsabilités

• First point of contact for walk-in, phone-based and third-party enquiries to Gold Coast Homelessness Service
• Conduct screening, triage and risk assessment; refer to case management and/or accommodation services
• Daily setup of the Gold Coast Homeless Hub drop-in service and follow up after hours inquiries
• Manage enquiries by phone, email, walk-in and third parties; use QHIP
• Capture required data on SHIP and QHIP systems
• Undertake intake and assessment to identify housing barriers and goals
• Coordinate access to daily drop-in services (breakfast, lunch, showers, laundry, phones, charging)
• Build rapport with individuals and families in crisis and demonstrate empathy and de-escalation
