Job summary

Apunipima Cape York Health Council in Cairns, QLD, is seeking a social and community worker with a focus on mental health and wellbeing. The role involves developing and delivering education and awareness activities tailored to client groups and coordinating through case management processes.

You will conduct assessments, maintain client data, liaise with service providers and contribute to culturally responsive care. A minimum of 2 years' experience and relevant certification are required.

Qualifications

• Minimum 2 years' experience as a social and community worker.
• Experience developing, implementing and reviewing education and awareness activities and programs based on client group needs.
• Well developed written and interpersonal communication skills including the ability to liaise with service providers.
• Knowledge of the case management process and the ability to undertake assessments, develop, monitor and review case plans.
• The ability to maintain client data and records including preparation of reports.
