Job summary

Queensland Health in Cairns is seeking an Aboriginal and Torres Strait Islander Advanced Health Worker to join the Central Community Care Team. The role focuses on delivering culturally sensitive health services to Aboriginal and Torres Strait Islander consumers in Far North Queensland, with emphasis on social and emotional wellbeing.

The position involves team-based assessments, treatment planning, and liaising with partner agencies to improve service coordination and outcomes.

Responsibilities

• Input and participate as a member of the team with assessments, therapeutic interventions, treatment and participate in case management, case reviews and planning providing cultural perspectives and advice.
• Liaise and network with the relevant agencies to enhance service delivery and encourage coordinated and effective services using effective communication and consultation skills.
• Be involved in the formulation, implementation and evaluation of the Service's strategies, plans, policies, and participate in continuous quality improvement activities working actively using the principles of effective team functioning.
• Linking with traditional healing practices.
