Job summary

Apunipima Cape York Health Council, Cairns, Queensland, is seeking an Elder Care Support Connector to work with local Elders and families to promote aged care entitlements and access to services.

You will connect communities to My Aged Care guidelines, organise supports, and help navigate complex service pathways while upholding culturally responsive practices and the principles of self-determination for Aboriginal and Torres Strait Islander peoples.

Qualifications

• Certificate IV in Community Services or Certificate IV in Community Development.
• Minimum two years' experience as a social/community worker.
• Experience supporting Aboriginal and Torres Strait Islander people.
• Current Blue Card or willingness to obtain prior to commencement.

Responsibilities

• Connect with local Elders to raise awareness of aged care entitlements.
• Organise and support access to appropriate aged care services.
