Job summary

Wellways is seeking a dedicated Aged Care Navigator to support Older Carers in navigating the My Aged Care system across Queensland, Southwestern Sydney and Nepean Blue Mountains. This full-time role based in Bowral requires travel to community sites and a valid driver’s licence.

Qualifications

• Tertiary qualifications in a related field and/or demonstrated relevant experience.
• Regional Assessment Service (RAS) or Aged Care Assessment Team (ACAT) assessor experience is highly desirable.
• Current valid Driver’s Licence.
• Appropriate IT skills.
• Evidence of working rights within Australia (birth cert/passport/visa).
• National Police Check (less than 12 months old).
• Blue Card and NDIS Worker Screening Check/Yellow Card QLD.
• NDIS Worker Orientation Module Certificate – free online course.

Responsibilities

• Support Older Carers and Carers of frail aged people, including CALD and Indigenous communities, to access the My Aged Care system.
• Collaborate with My Aged Care Assessment teams and contact centre staff to ensure Carer outcomes are met.
