Job summary

UnitingCare Queensland in Ipswich is seeking a Supported Living Facilitator for a casual role in Disability Services. You will support individuals in their daily living, with a focus on choice and quality of life, including personal care as needed.

You will work with families and networks to implement and review plans, promoting autonomy while respecting privacy and dignity. Sleepover shifts and weekend work are required.

Qualifications

• Experience in Disability, Aged and Community sector is ideal but not essential.
• Demonstrated ability to work in a supportive, person-centred environment.
• Commitment to safeguarding and confidentially.

Responsibilities

• Work with the person supported, family and networks to achieve life goals.
• Provide support in daily living including personal care with emphasis on choice.
• Develop, implement and review individual plans focusing on life hopes.
• Enable personal autonomy in accessing services and resources.
• Respect privacy, dignity and confidentiality of the person and family.
