Job summary

UnitingCare Queensland is seeking a Supported Living Facilitator to work with people with disabilities in Ipswich. In this casual role, you will support people in daily living, including personal care, and collaborate with families and informal networks to help them achieve their life goals.

The role requires flexibility for sleepover shifts and weekends, strong communication in English, and a current CPR/First Aid certification and NDIS Worker Screening.

Qualifications

• Must have current NDIS Worker Screening check.
• Open Australian Drivers License.
• CPR and First Aid certification required before start.
• Must be available for regular Sleepover Shifts and Weekend work.
• Able to communicate in spoken and written English.
• Experience in Disability sector is ideal but not essential; willingness to learn.

Responsibilities

• Work with the person supported, their family and networks to attain life goals.
• Provide support in everyday living, including personal care, with emphasis on choice and quality of life.
• Develop, implement and review individual plans focusing on hopes and aspirations.
• Enable autonomy over services and supports received.
• Uphold respect, privacy, dignity and confidentiality for the person and family.
