Job summary

Just Better Care is seeking a Clinical Care Partner to join our team in Frenchs Forest, NSW. You will conduct comprehensive nursing assessments, develop and monitor individualized care plans, and coordinate services with multidisciplinary teams to deliver high-quality in-home care.

You will lead non-clinical staff, ensure clinical oversight, and respond to evolving clinical risks. A strong focus on care management, culturally appropriate care, and trauma-informed practices is required.

Qualifications

• We are seeking a Clinical Care Partner to join our growing team and work with participants, families and health professionals to deliver safe, high-quality in-home care.
• NDIS Worker Screening Clearance / Criminal Record Check is required.
• Current Working with Children clearance is required.
• Valid Australian Driver's Licence and reliable vehicle (company car available for visits).
• Experience in case management or a Care Partner role is preferred.
• Strong knowledge of aged care, NDIS and the healthcare system.

Responsibilities

• Conduct comprehensive clinical assessments and document per standards.
• Develop detailed participant care plans by assessing needs, goals and preferences.
• Coordinate services with multi-disciplinary teams for seamless care delivery.
• Educate participants about their health care plans, rights and service agreements.
• Review and adjust care plans as needs change.
• Oversee participant budgets and ensure appropriate use of funding.
• Maintain accurate documentation and comply with standards and reporting.
• Oversee care plans and service coordination to ensure safe execution.
• Provide leadership to non-clinical Care Partners and guide on clinical risks.
• Train Support Workers to ensure they have necessary clinical skills.
• Collaborate with participants and supporters to address needs within funding guidelines.
• Contribute to continuous quality improvement initiatives.
