Job summary

Clinical Care Partner Community Services in Georges Hall, NSW, is seeking an experienced Registered Nurse to join the Community Services Team under the Support at Home Program. You will deliver person-centred clinical care, care coordination, education, and advocacy for older adults living at home.

The role involves coordinating in-home services, developing care plans, monitoring budgets, and building strong relationships with clients and external providers.

Qualifications

• Current AHPRA Registered Nurse registration.
• Experience in aged care, community nursing, or care management.
• Sound knowledge of the Support at Home Program and aged care standards.
• Strong communication, organisation, and time-management skills.
• Current NSW Drivers Licence with a reliable, insured vehicle.

Responsibilities

• Conduct comprehensive client assessments and develop care plans
• Coordinate in-home services and liaise with external providers
• Provide education, guidance, and advocacy for clients and families
