The Benevolent Society is seeking a Community-based Registered Nurse to provide in-home clinical support to older people across Western Sydney. You will deliver person-centred nursing care, assess needs, and work with multidisciplinary teams to respond to changing clinical requirements while promoting dignity and wellbeing.

Responsibilities include wound care, medication administration, and comprehensive assessments to support care planning, with a focus on independence and safety at home.

Qualifikationen

• 2+ years nursing experience within aged care, community care or disability settings.
• Valid First Aid & CPR certificate.
• Current AHPRA registration as a Registered Nurse is essential.

Aufgaben

• Provide in-home nursing care including wound care and continence management.
• Conduct clinical assessments and monitor client health.
• Deliver evidence-based nursing care aligned with aged care standards.
• Develop and evaluate holistic care plans.
• Coordinate with multidisciplinary teams and families for coordinated care.
• Promote service growth by building referral networks.
• Travel across Western Sydney to Penrith, Blacktown, The Hills District, Parramatta, Liverpool and Campbelltown.
