Job summary

The Benevolent Society seeks an Identified Case Manager to provide culturally safe, strengths-based intensive in-home support to at-risk families. You will use assessment tools to identify needs and develop tailored case plans.

Based in Loganholme with hybrid work options, the role requires Aboriginal and/or Torres Strait Islander identity, a relevant degree, and a current driver’s license. 12-month contract with potential for permanency.

Qualifications

• Bachelor’s Degree or Diploma in Social Work or similar field.
• Aboriginal and/or Torres Strait Islander identity and strong understanding of cultures and lived experiences of Indigenous children, families.

Responsibilities

• Provide culturally safe, intensive in-home family support to at-risk families.
• Undertake initial assessments and develop case plans.
• Collaborate with child protection and community services to improve safety and family functioning.
• Engage clients across ages, develop and review plans focusing on strengths and risk reduction.
• Coordinate with service providers to ensure access to resources.
