Russian Ethnic Representative Council in Victoria is seeking a Team Leader (Care Management) to lead and support a team of Care Advisors delivering Support at Home services. The role combines people leadership, operational management and clinical oversight to ensure safe, high-quality, culturally safe care aligned with standards.

Based in Ormond, this full-time position includes direct client case management alongside team leadership.

Qualifications

• Qualification in Nursing, Allied Health or other related field.
• Demonstrated leadership experience in community or aged care settings.
• At least 3+ years’ experience in HCP/ SaH care management.
• Knowledge of reforms in the Support at Home Program and broader aged care sector.
• Training in leadership, coaching, or quality improvement methodologies.
• Experience supporting CALD communities or diverse client groups.
• Fluent Russian

Responsabilités

• Provide effective day to day leadership, supervision, and professional guidance to Care Advisors, fostering a culture of accountability and client focus.
• Manage staff onboarding, professional development, and performance management.
• Identify training and coaching opportunities for staff to operate efficiently.
• Oversee daily operations of the SaH care management team to ensure timely service delivery.
• Ensure Care Advisors deliver services in line with program guidelines and quality standards.
• Maintain a small portfolio of SaH clients, providing direct case management and care planning.
