The role offers a 12-month max term contract covering Geelong and surrounding areas, with hybrid work and salary packaging options. You will work with experienced teams to improve independence and safety for clients in their homes.

Qualifications

• Must have qualification in case management or an undergrad qualification in nursing, allied health or social work.
• Experience with home care or community services is highly regarded.
• Ability to assess needs and plan care within a budget.

Responsibilities

• Conduct intake and ongoing assessments of aged care needs.
• Develop and maintain goal-directed care plans within the home care budget.
• Coordinate supports and services across the funded program.
• Manage package spend and advocate for customers’ needs and preferences; conduct regular home visits.
• Build strong relationships with customers, families and community stakeholders.
