Job summary

BaptistCare Victoria is seeking a Care Consultant to support clients in the Geelong region within our In-Home Care program. You’ll manage a home care caseload, plan services, monitor budgets and visit clients regularly to ensure care effectiveness.

The role emphasizes person-centred practice, autonomy, and a culture of care, with hybrid work and a 12-month max term contract. Applicants should be based near Mornington peninsula with travel as needed.

Qualifications

• Essential qualification in Case Management.
• Undergraduate qualification in Nursing, Allied Health, or Social Work.
• Experience in home care or community services is highly regarded.

Responsibilities

• Conduct intake, initial and ongoing assessments of aged care needs.
• Develop and maintain goal-directed care plans within each customer’s home care budget.
• Coordinate and facilitate supports across the funded program.
• Manage package spend and advocate for customers’ needs and preferences.
• Conduct regular home visits and monitor progress to support aging in place.
• Build strong relationships with customers, families and community stakeholders.
