Job summary
Ruah is seeking two Support Coordinators for its Housing First Support Service in Perth. The roles involve delivering person-centred case management and coordinating services to help clients achieve stable housing and better outcomes.

Qualifications
- Relevant qualifications at degree, diploma, or certificate level; or relevant lived experience
- Ability to develop person-centred support plans and goals
- Advanced report writing and analytical reasoning
- Strong ICT skills with Microsoft Office and client management systems
Responsibilities
- Deliver person-centred case management and support interventions
- Support clients to secure and sustain long-term housing
- Develop and monitor individual support plans and goals
- Collaborate with housing providers, services and government agencies
- Advocate for health, mental health, AOD, financial, legal and social supports
- Maintain accurate client records and case notes
- Manage competing priorities in a fast-paced service
- Contribute to a positive and collaborative team culture
