Job summary

Ruah is seeking two Support Coordinators for its Housing First Support Service in Perth. The roles involve delivering person-centred case management and coordinating services to help clients achieve stable housing and better outcomes.

This is a full-time, maximum-term role until 30 June 2027, 38 hours per week, Monday to Friday. You will work in a strengths-based, recovery-oriented framework with collaboration across housing providers and community services, advocating for health, AOD, financial

Qualifications

  • Relevant qualifications at degree, diploma, or certificate level; or relevant lived experience
  • Ability to develop person-centred support plans and goals
  • Advanced report writing and analytical reasoning
  • Strong ICT skills with Microsoft Office and client management systems

Responsibilities

  • Deliver person-centred case management and support interventions
  • Support clients to secure and sustain long-term housing
  • Develop and monitor individual support plans and goals
  • Collaborate with housing providers, services and government agencies
  • Advocate for health, mental health, AOD, financial, legal and social supports
  • Maintain accurate client records and case notes
  • Manage competing priorities in a fast-paced service
  • Contribute to a positive and collaborative team culture
