Job summary

United Foundation is seeking Home & Community Support Workers to deliver person-centred care in the homes of NDIS participants around Murray Bridge. You’ll help clients live independently, follow care plans, and maintain wellbeing through daily routines.

About The Role

Provide personalised care and support to participants in their own homes. Help clients live independently and safely while building meaningful routines.

  • Assist with grooming, toileting, cleaning, meals, and medication

  • Report concerns and changes in condition promptly

About You
  • Minimum 6 months' experience or Cert III/IV in Aged Care, Home & Community or Disability, or related field
  • Hands‑on experience in providing personal care (e.g. grooming, toileting, mobility assistance, meals, medication support)
  • Experience working in in‑home care
  • Strong understanding of professional boundaries, restrictive practices, and behaviour support
  • Effective written and verbal communication, including clear case note writing
  • Able to set priorities, meet deadlines, and work under pressure with minimal supervision
  • Strong interpersonal skills to work both autonomously and as part of a team
  • NDIS Worker Screening, First Aid, Driver’s Licence, and insured vehicle
