Job summary

Catholic Healthcare is seeking a Care Partner to provide relationship-centred care at home across the Northern Sydney area. The role will be office-based at Hunters Hill with visits to clients in the Northern Sydney region.

You will manage care plans, coordinate with clinical and allied teams, and ensure accurate billing and documentation while aligning services with aged care standards. This permanent full-time role offers a competitive hourly rate plus salary packaging.

Qualifications

• Experience in case management and care planning.
• Ability to communicate with clients and families about programs and funding.
• Proficiency with standard office software and electronic records.

Responsibilities

• Deliver relationship-centred care management for clients at home.
• Support effective care planning and funding utilisation.
• Educate clients and families on program guidelines and contributions.
• Coordinate with clinical and allied health teams to plan interventions.
• Use screening and risk dashboards to identify high-risk clients.
• Communicate and coordinate with team members to review outcomes of care.
