Job summary

Catholic Healthcare is hiring a Care Partner to provide relationship-centred care management and support clients to thrive at home. The role is office-based in Hunters Hill with travel to clients across the Northern Sydney area. It is a permanent full-time position, Monday to Friday.

You will focus on care planning, funding utilisation, and billing accuracy, while coordinating with clinical and Allied health teams to deliver high-quality outcomes for older people in the community.

Qualifications

  • Thorough understanding of the Aged Care Legislation and Aged Care Quality Standards.
  • Strong understanding of the ageing process, particularly age-related health issues.
  • Case management experience including assessment, care planning, goal writing and reviewing.
  • Previous experience working within a community setting in a similar role.
  • Ability to work independently and as part of a multi-disciplinary team.
  • Excellent digital literacy, including Microsoft Outlook and Excel.

Responsibilities

  • Achieving billable hours and service delivering expectations through effective care planning, funding utilisation, billing accuracy, and proactive management of client portfolio.
  • Undertaking person-centred care planning in accordance with Catholic Healthcare’s model of care.
  • Educating clients and their families on program guidelines, inclusions and exclusions and possible financial contributions.
  • Communicating and coordinating with members of the clinical, allied health and operational teams to identify client care needs, plan interventions, and review outcomes of care.
  • Using screening, assessment and risk dashboards to identify high risk clients and prioritize timely reviews.
  • Initiating case conferencing to support hospital, rehab or respite discharge planning for wellbeing interventions, service adjustments and timely referrals.
  • Promoting Catholic Healthcare Value proposition and service offerings while enabling clients to live at home independently with choice, control and connecting them to services that are safe, personal and effective.

  • $48.00 per hour + superannuation + Km reimbursement ( $1.11 per km)

Day to day this role looks like:

  • Ensuring accurate billing, timely documentation and alignment of services with client goals and funding availability
  • Understanding and adhering to the Aged Care Standards in all interactions, practices and documentation
To be successful in this role you’ll need:
