Job summary

Catholic Healthcare is seeking a Care Partner to provide relationship-centred care management and support clients to thrive at home across the Northern Sydney area. Office-based at Hunters Hill with visits to clients, Monday to Friday, delivering planned care and coordinating services to meet client goals.

You will apply knowledge of aged care legislation and standards, engage with clients and families, and document care plans, billing and service activity to ensure high-quality outcomes.

Qualifications

• Thorough understanding of Aged Care Legislation and Aged Care Quality Standards.
• Strong understanding of ageing and health issues in older adults.
• Case management including assessment, planning, goal setting and reviews.

Responsibilities

• Deliver relationship-centred care and manage client portfolios.
• Coordinate with clinical and allied health teams to identify needs.
• Educate clients and families on program guidelines and funding contributions.
• Ensure accurate billing and timely documentation aligned with goals.
• Promote Catholic Healthcare services and support home-based living.

Job description

• Location – Office based at Hunters Hill and visiting clients throughout the Northern Sydney area
• Permanent Full-Time role (Monday to Friday)
• $48.00 per hour+ superannuation + Km reimbursement( $1.11 per km)
• Up to $18,550 tax free with salary packaging

In the challenging and rewarding role of Care Partner, you will be providing relationship-centred care management that delivers exceptionalclientoutcomes,strengthens wellbeing, and supports clients to thriveand livesafely at home.

Day to day this role looks like:

• Achieving billable hours and service delivering expectations through effective care planning, funding utilisation, billing accuracy, and proactive management of client portfolio
• Undertaking person-centredcare planningin accordance withCatholic Healthcare’smodel of care
• Educating clients and their families on program guidelines, inclusions and exclusions and possible financial contributions
• Communicating and coordinating with members of the clinical, allied healthand operational teamstoidentifyclientcare needs, planinterventions, and review outcomes of care
• Using screening,assessmentand risk dashboards toidentifyhigh riskclients and prioritizetimelyreviews
• Initiating case conferencing to support hospital, rehab or respite discharge planning forwellbeing interventions, serviceadjustmentsandtimelyreferrals
• Communicating and coordinating with members of the clinical, allied healthand operational teamstoidentifyclientcare needs, planinterventions, and review outcomes of care
• Promoting Catholic Healthcare Value proposition and service offerings while enabling clients to live at home independently with choice, control and connecting them to services that are safe,personaland effective
• Ensuringaccuratebilling,timelydocumentationand alignment of services with client goals and funding availability
• Understanding and adhering to the Aged Care Standards in all interactions,practicesand documentation

To be successful in this roleyou’ll need:

• Thorough understanding of the Aged Care Legislation and Aged Care Quality Standards
• Strong understanding of the ageing process, particularly age-related health issues
• Case management experienceincludingassessment, care planning, goal writing and reviewing
• Previousexperience working within acommunity settingin a similar role
• Ability to work independently and as part of a multi-disciplinary team
• Excellent digitalliteracy, including Microsoft Outlook and Excel
