Job summary

Mercy Health is hiring a Community Care Partner in the South East region, a full-time role delivering in-home care management under the Support at Home Program. You will work within a rights-based, person-centred framework with participants, supporters and interdisciplinary teams to coordinate tailored, responsive, and inclusive support.

Qualifications

• Excellent time management, organisational and communication skills.
• A compassionate, flexible and positive approach to client care.
• Evidence of, or process of obtaining, a current NDIS Screening Check.

Responsibilities

• Care planning: reviewing care plans and budgets.
• Service planning and management with clients, supporters and carers.
• Monitoring, reviewing and evaluating care with a wellness and re-enablement focus.
• Providing support and education to clients and supporters to enable informed decisions.
