Qualifications

• Current Police Record Check and Working with Children Check required.
• Proof of influenza vaccination and other vaccinations as per public health orders.
• Experience with person-centred care is preferred.

Responsibilities

• Care Partner provides person-centred care management within the Support at Home Program, ensuring older people continue to live safely and independently at home.
