Job summary

Australian Nursing Home Foundation (ANHF) seeks a highly organised Care Partner to join our Support at Home Team in NSW. The role coordinates day-to-day administration of Support at Home Packages and delivers person-centred care to frail older clients and their carers.

This 12-month maximum-term full-time contract with view to permanency offers ongoing training, salary packaging options, and opportunities to work within a culturally diverse aged care community.

Qualifications

• Qualifications in aged care, disability, allied health, or related fields.
• Certificate IV in Care Support Services / Aged Care or Case management is desirable.
• Current First Aid and CPR certification is required.

Responsibilities

• Build and maintain relationships with clients and caregivers.
• Engage in community outreach to attract referrals and clients.
• Develop and review personalised care plans with clients and families.
• Coordinate services and activities of Support at Home Packages.
• Manage budgets and monthly statements.
• Provide direct care when needed and conduct home visits.
