Job summary

Care Connector (Holistic Nursing Solutions) invites applications for Registered Nurse (RN) and Enrolled Nurse (EN) roles in Brisbane, delivering home and community-based care to participants with complex needs.

You will work under experienced clinicians, provide safe, person-centred care in participants’ homes, coordinate with care plans, and build lasting professional relationships while advancing your specialised nursing skills in wound, continence, and spinal cord injury care.

Qualifications

• Current AHPRA registration.
• A current driver’s licence and reliable vehicle.
• Current National Police Check, or willingness to obtain one.
• Work rights in Australia.
• Experience in community or aged care is highly regarded.
• Experience in complex wound care and continence care is highly regarded.
• Experience with spinal cord injury care is highly regarded.
• Passion for client-centered care and service excellence.
• Excellent clinical judgement and attention to detail.
• Strong communication and a calm, professional approach.
• Flexibility to travel within the local area to support clients at home.
• Availability to start immediately is highly regarded.

Responsibilities

• Deliver safe, person-centred care to participants with complex needs.
• Support high-intensity clinical care (with training and supervision), including: spinal injury care, wound care, continence support and catheter care, medication assistance.
• Contribute to care planning and clinical oversight (RN).
• Support and guide support staff where required (RN).
• Follow care plans and positive behaviour support strategies.
• Monitor and report changes in participant condition.
• Complete documentation (progress notes, incident reports).
• Build strong, professional and respectful relationships with clients.
