Job summary

Care Connector (Holistic Nursing Solutions) is seeking Registered Nurses (RN) and Enrolled Nurses (EN) for community-based opportunities across Brisbane.

This is not high-volume agency work; you will deliver safe, person-centred care to participants with complex needs, supported by experienced clinicians in home and community settings.

Qualifications

• Current AHPRA registration
• Current National Police Check or willingness to obtain one
• Current driver’s licence and reliable vehicle
• Right to work in Australia
• Experience in community or aged care preferred
• Experience with complex wound care and continence care preferred
• Willingness to travel within local Brisbane area

Responsibilities

• Deliver safe, person-centred care to participants with complex needs
• Support high-intensity clinical care with training and supervision
• Contribute to care planning and clinical oversight (RN)
• Support and guide support staff as required
• Follow care plans and positive behaviour support strategies
• Monitor and report changes in participant condition
• Complete documentation (progress notes, incident reports)
• Build strong, professional relationships with clients
