Job summary

Care Connector (Holistic Nursing Solutions) seeks Registered Nurses (RN) and Enrolled Nurses (EN) for community-based opportunities across Brisbane. Deliver safe, person-centred care in participants’ homes and community settings, with support from experienced clinicians.

This role focuses on complex cases, wound and continence care, and spinal cord injury care, offering training, mentoring, and clear pathways for professional development. Immediate start is highly regarded.

Qualifications

• Current AHPRA registration is required.
• A current driver’s license and a reliable vehicle.
• Current National Police Check, or willingness to obtain one.
• Must have work rights in Australia.
• Experience in community or aged care is highly regarded.
• Experience with wound care and continence care is highly regarded.
• Experience with spinal cord injury care is highly regarded.
• Genuine passion for client-centred care and service excellence.
• Excellent clinical judgement and attention to detail.
• Strong communication and a calm, professional approach.
• Flexibility to travel within the local area to support clients in their homes.
• Availability to start immediately is highly regarded.

Responsibilities

• Deliver safe, person-centred care to participants with complex needs.
• Support high‑intensity clinical care with training and supervision (including spinal injury care, wound care, continence support and catheter care, medication assistance).
• Contribute to care planning and clinical oversight (RN).
• Support and guide support staff where required (RN).
• Follow care plans and positive behaviour support strategies.
• Monitor and report changes in participant condition.
• Complete documentation (progress notes, incident reports).
• Build strong, professional relationships with clients.
