Job summary

Care Connector (Holistic Nursing Solutions) is seeking Registered Nurses (RN) and Enrolled Nurses (EN) for community-based opportunities across Brisbane, Australia.

You will deliver person-centred care in participants' homes, supported by experienced clinicians, with emphasis on complex clinical cases, wound and continence care, and collaborative care planning. Flexible scheduling and ongoing professional development are provided.

Qualifications

• Current AHPRA registration is required.
• Driver’s licence and reliable vehicle.
• Current National Police Check or willingness to obtain one.
• Work rights in Australia.
• Experience in community or aged care is valued.
• Experience in complex wound care and continence care is valued.
• Experience with spinal cord injury care is valued.
• Passion for client-centered care and service excellence.
• Excellent clinical judgement and attention to detail.
• Strong communication and a calm, professional approach.
• Flexibility to travel within the local area to support clients at home.
• Availability to start immediately is highly regarded.

Responsibilities

• Deliver safe, person-centred care to participants with complex needs.
• Support high-intensity clinical care (with training and supervision), including spinal injury care, wound care, continence support and catheter care, medication assistance.
• Contribute to care planning and clinical oversight (RN).
• Support and guide support staff where required (RN).
• Follow care plans and positive behaviour support strategies.
• Monitor and report changes in participant condition.
• Complete documentation (progress notes, incident reports).
• Build strong, professional and respectful relationships with clients.

Care Connector (Holistic Nursing Solutions) is a clinician-led provider delivering holistic, person-centred care across the community.

• Complex and catastrophic injuries

About the Role
