Care Connector (Holistic Nursing Solutions) is seeking Registered Nurses (RN) and Enrolled Nurses (EN) for community-based opportunities across Brisbane, delivering safe, person-centred care in participants' homes and community settings. This is not high-volume agency work, with support from experienced clinicians and strong clinical leadership.

You will deliver care to participants with complex needs, participate in care planning, and contribute to ongoing clinical oversight while building

Qualifications

• Current nursing registration in Australia.
• Experience delivering care in participants' homes.
• Ability to work independently within community settings.
• Strong interpersonal and communication skills.

Responsabilités

• Deliver safe, person-centred care to participants with complex needs.
• Support high‑intensity clinical care (with training and supervision).
• Contribute to care planning and clinical oversight (RN).
• Support and guide support staff where required (RN).
