Job summary

Lifecare Homecare is seeking a dedicated Occupational Therapist to deliver person-centred assessments and interventions in clients’ homes and in collaboration with a multidisciplinary team across Victoria. You will prepare reports for funding bodies and contribute to achieving meaningful client outcomes.

The role supports older adults and people living with disability, with flexible full- or part-time options and opportunities for professional development within the Lifecare/APM Group.

Qualifications

• Occupational Therapy degree with current General AHPRA registration.
• Ability to work effectively within a multidisciplinary team while managing a community-based caseload independently.
• Genuine interest in supporting older adults and people living with disability.
• Full working rights in Australia.
• Current driver’s licence, access to a reliable vehicle and comprehensive car insurance.
• Current Working with Children Check and NDIS Worker Screening Clearance, or ability to obtain them.
• Willingness to undergo a National Criminal History Check as part of recruitment.

Responsibilities

• Deliver high-quality Occupational Therapy services tailored to each client’s individual needs using a person-centred approach.
• Conduct assessments and provide interventions for clients within their home environment.
• Collaborate with Case Managers, Allied Health Professionals, GPs, and other healthcare providers to ensure effective client outcomes.
• Prepare high-quality reports that meet funding body requirements (NDIS, DVA, Medicare, Home Care Package).
• Demonstrate Lifecare Homecare’s core values of Care, Respect, Integrity, and Service Excellence.
• Build and maintain strong relationships with Case Managers, GPs, community centres and other referral partners.
