Lifecare Homecare in Melbourne is seeking a full time or part time Occupational Therapist to provide high-quality assessments and interventions to support independent living. You will work in clients' homes, tailoring services to individual needs within a multi-disciplinary team.

Responsibilities include home assessments, client-centred interventions, collaboration with Case Managers, GPs, and funding bodies, and reporting for NDIS, DVA, Medicare and Home Care Package providers.

Qualifications

• Must hold Occupational Therapy degree and General AHPRA registration.
• Ability to work in a multidisciplinary team and manage a community-based caseload.
• Interest in supporting older adults and people living with disability.
• Full working rights in Australia.
• Current driver’s licence, access to a vehicle and comprehensive car insurance.
• Current Working with Children Check and NDIS Worker Screening Clearance, or ability to obtain.
• Willingness to undergo National Criminal History Check.

Responsabilités

• Provide high-quality OT services in clients' homes using a person-centred approach.
• Conduct home assessments and provide interventions tailored to client needs.
• Collaborate with Case Managers, Allied Health Professionals, GPs and other providers to achieve outcomes.
• Prepare reports meeting funding body requirements (NDIS, DVA, Medicare, Home Care Package).
• Demonstrate Lifecare's values in all work and build relationships with referral partners.
