Job summary

Lifecare Homecare in Melbourne is seeking a full time or part time Occupational Therapist to provide high-quality assessments and interventions to support independent living. You will work in clients' homes, tailoring services to individual needs within a multi-disciplinary team.

Responsibilities include home assessments, client-centred interventions, collaboration with Case Managers, GPs, and funding bodies, and reporting for NDIS, DVA, Medicare and Home Care Package providers.

Qualifications

• Must hold Occupational Therapy degree and General AHPRA registration.
• Ability to work in a multidisciplinary team and manage a community-based caseload.
• Interest in supporting older adults and people living with disability.
• Full working rights in Australia.
• Current driver’s licence, access to a vehicle and comprehensive car insurance.
• Current Working with Children Check and NDIS Worker Screening Clearance, or ability to obtain.
• Willingness to undergo National Criminal History Check.

As a full time or part time Occupational Therapist you will be providing high-quality assessments and interventions to our clients to support independent living. This encompasses ensuring effective and efficient assessment, planning, interventions and evaluation in a multi-disciplinary setting.

In this role, you will…

• Deliver high-quality Occupational Therapy services tailored to each client’s individual needs, using a person-centred approach.
• Conduct assessments and provide interventions for clients within their home environment.
• Collaborate with Case Managers, Allied Health Professionals, GPs, and other healthcare providers to ensure effective client outcomes.
• Prepare high-quality reports that meet the requirements of funding bodies, including NDIS, DVA, Medicare, and Home Care Package providers.

• Build and maintain strong relationships with Case Managers, GPs, community centres, and other key referral partners.
