Job summary

Western Sydney Local Health District seeks an experienced Change Management leader to drive adoption of new care models. You will work with clinical, operational and project leaders to support the Out of Hospital Care Model, ensuring effective stakeholder engagement, communication, training and benefits realisation across hospital and community settings.

You will design and embed comprehensive change plans, manage risks, run workshops, and collaborate with the Project Management Office to align

Qualifications

• Tertiary qualifications in change management or related certification.
• Proven track record in change management within large, complex service environments.
• Experience in hospital or health setting preferred but not essential.
• Excellent communication skills (written and verbal) with ability to present to diverse audiences.
• Proven delivery of end-to-end management of multiple integrated communication campaigns.
• Strong stakeholder engagement and relationship management with multidisciplinary teams.

Responsibilities

• Lead the change management function for the Out of Hospital Care Model and related initiatives.
• Develop and implement change management, stakeholder engagement, communication, training and benefits realisation plans.
• Partner with clinicians, operational leaders and project teams to assess change impacts, build readiness and support adoption.
• Facilitate workshops, briefings and engagement activities to drive understanding, commitment and sustainable behavioural change.
• Monitor change progress, manage risks and provide expert advice to support project and organisational outcomes.
