Job summary

Swan Hill District Health is seeking a part-time Healthcare worker to join the Support at Home team. The role focuses on community-based services, offering 1:1 supervision in clients' homes and delivering individual care plans to help clients live independently for as long as possible.

Working hours are 0730 hrs to 1530 hrs. You bring a Cert III in Individual Support, a current driver’s license, and a commitment to diversity and client dignity.

Job description

A Part time position is available for a Healthcare worker to join our enthusiastic team of Health Professionals in the Support at home team at Swan Hill District Health.

The Position

This role is community based and aims to support people to live as independently as possible for as long as possible through the provision of home support services, including personal care and flexible respite services to support carers. The Health Care Workers aim is to provide 1:1 supervision of consumers within their own home with Swan Hill District Health to ensure a safe environment for consumers and staff. Working hours are 0730 hrs to 1530hrs

You Bring

• Completion of at least a Cert III in Individual support or its equivalent that is relevant to the field of practice.
• Understanding of the duties and commitment to older people to maintain client independence in continuing to live Safely in their home
• Must have a Current valid Victorian driver’s license
• The ability to carry out a broad range of tasks, according to the rostered service provision and care/service plan that are socially and culturally appropriate and free from discrimination.
• Ability to maintain confidentiality
• Good communication skills including written, verbal and listening
• A flexible approach to delivery of a range of client services

Mandatory requirements

• Current National Police Check, or will to obtain.
• Working with Children’s Check, or will to obtain.
• NDIS worker screening check, or will to obtain.
• Evidence of immunisation records/history as part of the Health Services Act 1988, 2020 Amendment (Mandatory Vaccination of Healthcare Workers).
