Job summary

NSW Health's Westmead Hospital is seeking a Level 1/2 Social Worker to provide professional social work services to older patients in the Geriatric Medicine Department. You will assess, counsel, advocate and connect families with appropriate services to support care planning.

Qualifications

• Understanding of ageing, age-related illnesses, rehabilitation, guardianship and end-of-life care.
• Unrestricted NSW Driver’s Licence and ability to participate in the After Hours Crisis On-Call Social Work Service.
• Good knowledge of community resources and experience liaising with community organisations.
• Clinical experience or understanding of grief and loss counselling, crisis intervention, discharge planning and working with complex family dynamics.
• Understanding of issues associated with culturally and linguistically diverse client groups.
• Strong time management with excellent written and verbal communication, including report writing and high computer literacy.
• Ability to work with minimal supervision and as part of a multidisciplinary team.
• Commitment to quality improvement and ongoing clinical learning and practice.

Responsibilities

• Provide professional social work services to older patients, families and carers under the Geriatric Medicine Department.
• Conduct psychosocial assessments and deliver counselling, crisis intervention, advocacy and referrals to community supports.
• Support patients and families with complex challenges, including elder abuse and guardianship matters.
• Collaborate with multidisciplinary teams to support coordinated care and discharge planning.
• Maintain clinical documentation and participate in quality improvement initiatives, plus education for healthcare colleagues.
• Pursue professional development via supervision and evidence-based practice; support social work students when required.
