Job summary

NSW Health's Ryde Hospital is seeking a Social Worker Level 3 to join a dedicated team caring for older people, their families and carers. This permanent, full-time role focuses on complex discharges on a general medical ward, with cover for ED during business hours on a roster basis.

Qualifications

• A Bachelor or post graduate degree in Social Work and eligibility for the Australian Association of Social Workers (AASW).
• Evidence of extensive clinical experience, inclusive of experience as a Level 2 Social Work.
• Demonstrated ability to initiate, lead, complete and share quality improvement activities or service evaluation process.
• Experience in clinical teaching and supervision of students, level 1/2 Allied Health Professionals and/or Allied Health Assistants.
• Demonstrated proficient skills and knowledge in the provision of current social work practice with a focus on discharge planning.

Responsibilities

• Work on a general medical ward with a focus on complex discharges.
• Provide cover to ED during business hours on a roster basis.
• Contribute to quality improvement and service evaluation activities.
