Job summary

Northern Sydney Local Health District is seeking a Social Worker Level 3 at Ryde Hospital to support older people, their families and carers with complex discharge planning and comprehensive social work assessments.

The role operates across acute wards and rehabilitation, with discharge planning as a focus and rostered ED coverage during business hours. Aboriginal and Torres Strait Islander people are encouraged to apply.

Qualifications

• Bachelor or post graduate degree in Social Work and eligibility for the Australian Association of Social Workers (AASW). Evidence of extensive clinical experience, inclusive of experience as a Level 2 Social Work.
• Demonstrated ability to initiate, lead, complete and share quality improvement activities or service evaluation processes; experience in clinical teaching and supervision of students and allied health staff.
• Proficient skills and knowledge in current social work practice with a commitment to evidence-based service for consumers, carers and families.

Responsibilities

• Provide social work services on a general medical ward with a focus on complex discharges.
• Cover ED in business hours on a roster basis as required.
