Job summary

NSW Health’s Social Worker Level 1/2 in the RACE Team of Geriatric Medicine provides psychosocial care for older people and their families across hospital and community settings. You will collaborate with medical, nursing and allied health staff to ensure person-centred care, advocacy, and support for complex psychosocial needs.

Qualifications

• Experience in psychosocial care for older people and families.
• Ability to liaise with community organisations and services.
• Ability to conduct comprehensive psychosocial assessments and interventions.
• Demonstrated understanding of elder abuse, guardianship, and end-of-life care.
• Experience working in multidisciplinary teams and in hospital/community settings.
• Excellent written and verbal communication and report writing skills.

Responsibilities

• Deliver psychosocial assessments and interventions, including counselling and referrals.
• Support clients through complex and sensitive situations (elder abuse, guardianship, loss).
• Collaborate in multidisciplinary care planning and ward rounds.
• Maintain accurate clinical documentation and participate in quality improvement initiatives.
• Engage in ongoing professional development and supervision.
