Job summary

Mater Health Services in Brisbane is seeking a Clinical Nurse to join the Elder Care Team on a permanent, part-time basis (24 hours per week). You will provide specialist nursing assessment, discharge coordination and leadership for older patients in ED, AMU and GEM, focusing on frailty and geriatric syndromes to improve outcomes.

The role requires current AHPRA registration as a Registered Nurse, experience in emergency or geriatric care, and strong communication and teamwork to support safe

Qualifications

• Current AHPRA nursing registration as a Registered Nurse (no restrictions).
• Experience in emergency nursing or geriatric care preferred.
• Compassion and respect for older people and their carers.
• Strong assessment, communication, and clinical decision-making skills.
• Commitment to patient-centred care and interdisciplinary teamwork.

Responsibilities

• Contribute to comprehensive geriatric assessments for older adults with complex health needs.
• Identify, assess and coordinate management of geriatric syndromes including frailty, delirium, dementia, falls risk, functional decline, malnutrition and polypharmacy.
• Develop and implement individualised, goal focused, care plans in collaboration with physicians, allied health professionals, and family members.
• Provide education and emotional support to patients, families and carers regarding health conditions, treatment plans and available services.
• Support continuity of care by liaising with community services and primary care providers.
• Completing referrals for assessments in the home through services such as My Aged Care.
• Work with Medical and Allied Health Teams to assess safety for discharge home or admission to hospital in a timely manner.
• Act as a clinical resource and advocate for geriatric patients within various settings of the hospital.
• Participate in quality improvement initiatives and staff education related to geriatric care.
• Support evidence-based management of Behavioural and psychological symptoms of dementia (BPSD/Changed Behaviours).
• Support smooth transitions between acute, subacute, rehabilitation, residential aged care and community settings.
• Contribute to discharge coordination to minimise readmission risk and optimise patient’s outcomes.
