Job summary
Mater Hospital Brisbane is seeking a qualified Clinical Nurse to join the Elder Care Team on a permanent part-time basis (24 hours per week). You will provide specialist nursing assessment, discharge coordination and leadership for older people in the Emergency Department, Acute Medical Unit and GEM ward.

Qualifications
- Current AHPRA nursing registration as a Registered Nurse.
- Experience in emergency nursing or geriatric care preferred.
- Commitment to patient-centred care and interdisciplinary teamwork.
- Strong assessment, communication and clinical decision-making skills.
Responsibilities
- Contribute to comprehensive geriatric assessments for older adults with complex health needs.
- Identify, assess and coordinate management of geriatric syndromes including frailty and delirium.
- Develop and implement individualised, goal-focused care plans with the team.
- Provide education and emotional support to patients and families regarding health conditions and plans.
- Support discharge coordination to minimise readmission risk and improve outcomes.
- Act as a clinical resource and advocate for geriatric patients across hospital settings.
