Job summary

Tender Loving Care Aged Care – Bankstown is seeking a Care Partner to support customers through their care journey, coordinating services and developing holistic care plans to help them live safely at home or in a preferred setting.

You will build strong relationships, conduct onboarding assessments, and work with clinical teams and families to ensure person-centred care and smooth transitions across health needs.

Qualifications

• Experience in community, aged care, disability or health services, including case management and coordinating multidisciplinary care.
• Strong customer focus and empathy in sensitive conversations.
• Excellent communication and proficiency with Microsoft Office and customer management systems.
• Knowledge of funding streams, quality standards, audit requirements and care transitions.
• Ability to work across multidisciplinary teams in a fast-paced environment.

Responsibilities

• Build trusted relationships with customers and support them to achieve their goals at home.
• Conduct onboarding assessments, develop holistic care plans, coordinate multidisciplinary services and complete customer reviews as needs change.
• Collaborate with clinical teams, families and external providers to deliver safe, person-centred care and achieve optimal customer outcomes.
• Support customers through care transitions, including hospital discharge, changes in health needs and offboarding activities where required.
• Identify unmet needs, optimise funding utilisation and support access to services that enhance customer outcomes.
• Maintain accurate customer records in enterprise systems and contribute to quality, compliance and continuous improvement activities.
• Promote safety, quality and person-centred care across all aspects of service delivery.
