Job summary

Northside Community Service is seeking a Care Partner (Registered Nurse) to deliver community-based nursing in clients’ homes under the Support at Home program. You will provide clinical nursing support, assess needs, and coordinate services to keep older people safe and independent.

The role emphasizes person-centred, wellness and reablement-focused care within a supportive multidisciplinary team. Existence of driver requirement and screens indicated.

Qualifications

• Current, unrestricted registration as a Registered Nurse with AHPRA.
• Extensive post-registration experience in aged care, home care or community health.
• Experience in goal-oriented care coordination, care planning and ongoing monitoring.

Responsibilities

• Deliver nursing care in clients’ homes, including wound care and medication management.
• Conduct clinical assessments and develop/review care plans.
• Coordinate services, referrals and onboarding in line with care plans.
