Qualifications

• Empathy, planning and problem-solving skills to support older adults living at home.
• Strong communication and relationship-building abilities with participants and families.
• Ability to balance client advocacy, funding requirements and compliance.

Responsibilities

• Translate participant goals into personalised, compliant care plans.
• Regularly review and update care plans as needs change.
• Plan and track participant budgets to achieve goals and value for money.
• Guide participants through informed decisions on services and funding.
• Coordinate services and referrals to ensure seamless care.
• Maintain accurate, audit-ready documentation.
