Job summary

Anglicare Sydney is seeking a dedicated Tailored Support Coordinator to assess resident needs and work with individuals to develop practical, achievable support plans in collaboration with residents and service providers.

You will connect residents with health, legal, financial, advocacy and Anglicare services, coordinate case discussions, monitor progress and maintain accurate, confidential records to support wellbeing, social participation and independence.

Qualifications

Bachelor-level qualifications in social work or related field; experience in community services preferred.
At least 12 months post-qualifying experience delivering community-based social services.
Experience coordinating services for people with complex needs and maintaining case documentation.
Strong communication, advocacy and stakeholder relationship skills.

Responsibilities

Complete resident needs assessments and develop tailored support plans with clear, achievable outcomes.
Connect residents with health, legal, financial, advocacy, community and Anglicare services.
Coordinate case discussions and build productive relationships with residents, families and service providers.
Monitor support plans, resident progress, service quality and budgets, adapting as needs change.
Maintain accurate and confidential case records and contribute to wellbeing and participation reporting.
