Job summary

Anglicare Sydney is seeking a Tailored Support Coordinator to assess resident needs and connect them with services to improve health, wellbeing and independence. Reporting to the Senior Tailored Support Coordinator, you will develop practical support plans and coordinate care across health, legal, financial and community services.

You’ll bring experience in social services, strong coordination and advocacy skills, a car license, and a commitment to integrity and compassionate service.

Qualifications

• Strong coordination and relationship-building skills to connect residents with services.
• Ability to communicate effectively with residents, families and service providers.
• Experience coordinating services for people with complex needs and maintaining case documentation.

Responsibilities

• Complete resident needs assessments and develop tailored support plans with clear outcomes.
• Connect residents with health, legal, financial, advocacy, community and Anglicare services.
• Coordinate case discussions and build productive relationships with residents, families and service providers.
• Monitor progress, service quality and budgets, updating records as needs change.
• Contribute to wellbeing, participation and tenant satisfaction reporting.
