Job summary

UnitingCare Queensland is seeking a Casual Supported Living Facilitator to support clients in Plainland and surrounding areas. You will travel between homes, delivering in-home care and daily living assistance as part of the FaDS team.

Role involves community-based activities, driving between visits, and coordinating with families to implement Care Plans. CPR/First Aid and NDIS checks required before start.

Qualifications

• In home personal care and daily living assistance.
• Must have a current driver’s licence and access to a vehicle.
• NDIS Worker screening check is required.

Responsibilities

• Provide in-home care to clients to support independence and daily living.
• Drive between clients in your own vehicle to scheduled visits.
• Assist with community activities and transport to appointments.
